Diabetes is a life-long disease affecting 24 million Americans. Diabetes impairs the body’s utilization of nutrients. It can result in uncontrolled blood sugars and abnormal lipids, leading to both short-term and long-term complications (including heart disease and stroke) if it goes unchecked.
The diabetes program is an American Diabetes Association Recognized Program. It includes a multi-disciplinary staff of diabetes educators, including a Spanish–speaking educator. We serve adults with pre-diabetes as well as those with type 1 or type 2 diabetes. Participants are taught self-management skills and are encouraged to implement self-care behaviors to minimize the risk of complications and improve their overall well-being.
Our program includes—
- A three hour group education class which provides an overview of diabetes self-management
- A two hour individual consultation with a registered nurse and registered dietitian
- Two follow-up visits
- A bi-monthly continuing education program to keep you abreast of the latest guidelines and practices
